We are seeking a dedicated Patient Care Coordinator to join our hospice team. The ideal candidate will play a crucial role in ensuring the smooth operation of our office and providing exceptional patient care.
- Duties
- Greet and assist patients in a friendly and professional manner over the phone
- Schedule CNA visits for our patients
- Maintain electronic health records accurately
- Maintain our local CNA schedules and assignments
- Provide CNA coverage if necessary
- Answer phone calls, emails, and inquiries promptly
Experience
Required Skills:
- Active CNA license for the State of Texas
- Hands-on experience with office equipment (e.g. fax machines and printers)
- Professional attitude and appearance
- Solid written and verbal communication skills
- Ability to be resourceful and proactive when issues arise
- Excellent organizational skills
- Multitasking and time-management skills, with the ability to prioritize tasks
- Customer service attitude
- High school degree; additional certification in Office Management is a plus
