Job Summary:
The RN Case Manager works with families and other healthcare professionals during the final stages of patients’ lives. Primary functions are to administer nursing care for patients in their place of residence, residential facilities, nursing homes, and other hospice care environments. Registered Nurses will coordinate care with the interdisciplinary team, patient/family and referring agency. Registered Nurses must be capable of compassionate communication during visits with patients and families. Will work under general supervision of the Director of Nursing and Hospice Physician. Registered Nurse must have excellent observation skills and high ethical standards. Registered Nurse may supervise LVNs and instructs non-professional hospice aide staff in the provision of personal care to the patient.
Job Responsibilities:
- RN Case Manager will provide leadership and oversight of clinical activities, that includes, but not limited to, assess and evaluate patient needs/problems, individualized care plan development, care coordination, on-site field visits and case management.
- Promotes hospice core values, mission, goals, and philosophy.
- Participates in care coordination to ensure proper communication between caregivers, patients, and physicians.
- Perform timely and appropriate documentation, quality of care, care coordination between disciplines, and the supervision of Hospice Aides.
- Perform the assessment and reassessment of patients, including updating of care plans and interpreting patient needs, while adhering to Agency, physician, and/or health facility protocols.
- Maintains a high level of knowledge pertaining to the management of pain and symptoms in the dying process
- Maintains close contact with the patient’s family and/or significant others to provide information, support, and continuity of care.
- Practices infection control measures and standard precautions.
- Works closely with other members of the Interdisciplinary Group to provide all-inclusive physical, psychosocial, educational, emotional and spiritual services including bereavement support to the patient and family to achieve the highest quality of care.
- Reports any change in the patient’s condition to both the Attending Physician and Hospice Physician.
- Performs other activities as assigned.
- Completes required annual training.
- Timely documentation is mandatory
- Proficient in Wellsky and/or Consolo Electronic Medical Record Software
- Willing to cover a flexible mile radius
Qualifications:
- Must be a graduate of an approved school of Nursing.
- Current Texas State license as a Registered Nurse.
- The Registered Nurse will have 2 years Hospice nursing care experience (Required)
- Effective communication skills
- Excellent Time management Skills
- Current Texas Driver's License.
- Must have and maintain an automobile to be used for work.
Physical and Mental Effort:
Prolonged standing and walking required, with ability to lift up to 50 lbs and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient/family individualized psycho social needs. Requires hand-eye coordination and manual dexterity
