Job Summary
We are seeking a dedicated and compassionate Registered Nurse and Licensed Vocational Nurse to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including patient homes and assisted living facilities. This role requires a strong commitment to patient advocacy, effective communication skills, and the ability to work collaboratively with multidisciplinary teams.
Duties
- Assess and monitor patient health status, including vital signs and medical history.
- Administer medications and treatments as prescribed by physicians.
- Develop and implement individualized care plans for patients, particularly those with dementia or disabilities.
- Provide education and support to patients and their families regarding health management, including family planning options.
- Utilize electronic health record systems such as MatrixCare for documentation and patient management.
- Collaborate with other healthcare professionals in hospice medicine to ensure comprehensive care delivery.
- Maintain a safe environment for patients by adhering to infection control protocols and safety guidelines.
Skills
- Proficiency in using electronic health record systems (MatrixCare).
- Experience in dementia care and working with individuals with disabilities.
- Knowledge of family planning practices, bereavement and patient education techniques.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
- Ability to work in fast-paced environments while maintaining attention to detail.
