Job Description
Essential Job Functions:
- Screen, schedule, and facilitate health care navigation for patients referred to the program.
- Document treatment plans and communicate patient treatment needs across caregivers and internal/external providers.
- Provide patient care management services including regular outreach and resource allocation; tracking youth progress and treatment needs over the course of 6-12 months.
- Maintain a patient registry and rigorously document patient health outcomes.
- Enable and coordinate multidisciplinary teams.
- Provide operational and administrative support for Collaborative care and Stronger Together initiatives and programs, including meeting planning and coordination, maintenance of all project materials and tracking project activities.
- Facilitate collection, tracking and reporting of data and other project activities in a timely manner to demonstrate impact for internal reporting, grant applications, stewardship reports and other related requests.
- Build and maintain relationships with internal and external stakeholders.
Knowledge, Skills and Abilities:
- Bachelor’s degree in psychology or mental health-related sciences or related field. Master’s degree in mental health preferrable.
- Excellent interpersonal, verbal, and written communication skills.
- Strong organizational skills, including management of time and processes.
- Ability to take initiative, collaborate with clinical teams, be self-directed.
- Ability to collaborate with individuals from different backgrounds and disciplines.
- Commitment to health equity, diversity and inclusion, racial justice, evidence-informed policy and practices, data-driven advocacy and community-driven processes.

Pay Range
$59,280.00-$96,928.00 Salary

The pay range for this job indicates minimum and maximum targets for the position. Ranges are regularly reviewed to stay aligned with market conditions.
