Position Overview:
- Provide in-home care to pediatric or adult patients with complex needs, including G-Tubes, Trachs, and ventilators.
- shifts, Day /night shifts, Monday to Sunday
- Locations: San Fernando 91340
- The duties of a shift LVN include administering medication, managing chronic illnesses, assessing changes in patient’s medical status, completing care documentation, and helping with daily living activities.

As a Home Health Licensed Vocational Nurse(LVN), what you’ll do:
Responsibilities:
- Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN(Registered Nurse)
- Licensed Vocational Nurse ( LVNs)will be given a detailed plan of care instructed and administered by RN Case Manager
- Care consists of monitoring patient status, maintaining activities of daily living and administering medications.
- Take patient vitals, do assessments, document progress and report changes to RN supervisor.
- Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques
- Collaborates with physicians, other agency staff and contract services to coordinate and implement the Plan of Care.
- Completes all required documentation in accordance with the standards of the agency, and regulatory requirements.

Requirements:
Current CA state license to practice as a Licensed Vocational Nurse in California.
A minimum of one (1) year home health experience is strongly preferred.
Current BLS/CPR Certification
Experience in caring for patients with G-Tubes, Trachs, and ventilators.
Works in patients home in various conditions; ability to work a flexible schedule.
Able to travel to prospective patient’s place of residence.
Able to stoop and bend; must be able to lift and transfer patients.
