Continuity Care Home Nurses is actively recruiting for RNs, LVNs, and CNAs with our growing patient base. You will be working in the home providing care to our clientele many of whom have special skills including brain injury, paraplegic patients, ventilator patients, trach patients, etc. We offer great benefits and 401k with match. Qualifications :
Must have one year of clinical experience, facility or rehab experience preferred
Certified CNA
Licensed LVN or RN
NO NEW GRADS, please
CPR Certification - American Red Cross or American Heart Association
Physical / TB test
2 work references
Valid I9 Documentation
Compassionate and caring demeanor for our patient base
Responsibilities :
Care for ill, injured, or convalescing patients requiring special skills such as brain injury, paraplegic patients, ventilator patients, trach patients
Provide basic patient care and treatment
Collaborate with registered nurses to administer prescribed medications and follow patient’s specific plans of care
Educate and update patients and families on medical treatments
Accurate and timely documentation skills are necessary

- Hi-Tech RN : $55-75 / hr
RN : $45-75 / hr
LVN : $28-$40 / hr
CNA : $17.28-25 / hr

Our personalized service, knowledgeable staff, and close monitoring of services provided create a warm, supportive, and caring environment for patients as well as family members. We provide Certified Nurse’s Aides & skilled nursing care (RNs, LVNs).... from four hours / day to 24 hours / day. Our second company Continuity Care Staffing Services, Inc. provides babysitting & caregivers to pediatric and geriatric patients.
We specialize in high tech home infusion visits & care to catastrophic patients (Ventilator Dependent, Spinal Cord Injury, Brain Injured) in addition to having PT, OT, ST, LCSW available. All cases are supervised by Registered Nurses who also case manage each patient. We can bill all private insurance plans, worker's compensation, long term care insurance, & accept all major credit cards. Our service area includes most of Southern CA.
