JOB SUMMARY
Provides support to independent practices to reduce and eliminate patient and practice disparities. Reviews/analyzes data to identify the effectiveness of existing processes and implement screening and intervention procedures for practices regarding Social Determinants of Health and Social Needs. Engages with practice operational and clinical leadership to develop sustainable changes. Collaborates with analysts to develop actionable performance reports. Generates measurable improvements in clinical performance and patient experience. Shares best practices across member practices through individual meetings and monthly work groups.
ESSENTIAL FUNCTIONS
Duties and responsibilities:
1. Provide face-to-face and telephonic interaction with practice contacts to support Social Determinants of Health/Social Needs Screenings.
2. Direct patient outreach to provide information and support accessing community resources.
3. Patient education and advocacy around preventive and chronic disease management.
4. Work with partnered practices to develop and maintain a patient-centered community resource guidebook.
5. Support the development, implementation, monitoring of evidence-based care screening, and intervention methods to meet organizational goals.
6. Coordinate with the PO Informatics Team to provide insight to PO projects and improvement activities.
7. Assist with practice workgroups, meetings, and webinars related to community health work.

7. Competency in using various computer hardware and software systems (e.g., Microsoft Office).
Required Qualifications:
1. High school diploma, additional education or training in public health, social work, or related field preferred.
2. Previous experience with health care advocacy, health promotion, or health education
3. Experience working with diverse, vulnerable, and sensitive populations
4. Strong knowledge of community assets and local resources
5. Current certification as a Community Health Worker or ability to complete training by June 30, 2025 through Answer Health training and development.
Preferred Qualifications:
· Current certification as a Community Health Worker
· Previous experience working with a network, Physician Organization, Physician-Hospital Organization, or Independent Practice Association.
Work Environment
Office Environment: Office is a professional office setting with individual offices, common area desks, and conference rooms. The noise level in the work environment is usually low to moderate.
Physical Effort: The job includes limited physical demands, typical of the job, which include regular walking, standing, stooping, bending, sitting, typing, using the phone, and lifting light objects.
Mental & Visual Fatigue: Work may produce mental/visual fatigue. Detailed, interactive work requiring alertness and concentration for sustained periods, operation and complete attention to a computer.

Pay: $19.00 - $22.00 per hour
Expected hours: 40 per week
