What You'll Do
As a Patient Care Manager in a home setting, you will work closely with patients, families, and healthcare providers to create personalized care plans that meet individual needs. Your responsibilities will include:
- Providing direct patient care and treatment under the supervision of a Registered Nurse
- Ensuring high-quality patient experience and positive health outcomes by delivering the plan of care established by the RN Case Manager
- Preparing complete and timely clinical notes, coordinating care delivery with an interdisciplinary team (PT, OT, ST, MSW, and HHA)

Requirements
- Current CA Licensed Vocational Nurse (LVN) license
- Current IV Certification
- Current CPR card for Healthcare Providers
- 1 year of Licensed Vocational Nurse (LVN) experience
- Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order
