Coordinates palliative and referrals and services, as ordered by physicians, following department standards, for patients with community resources, communicating effectively with community agencies and service providers to facilitate care coordinationand information sharing.
Coordinates and arranges visit appointments for providers and patients for patient and family meetingsAssist in obtaining Palliative orders for clinically qualified patients and after approval from patient or patient's legal representative.Coordinate co-visits with Home Health nurse once Palliative orders obtainedCommunicate in an active, positive and effective manner to all inter-professional health care team members and reports pertinent patient care and family data in a comprehensive and unbiased manner, listens and responds to the ideas of others.Works collaboratively and maintains active communication with physicians, nurses, office staff and other members of the multidisciplinary care team to resolve problems that are identified during follow-up phone calls.Documents visits / activities as required by program type, statistical forms and procedures and maintains complete patient records while keeping complete patient confidentiality.Actively participates in quality improvement initiatives and collects data for use in departmental performance improvement as directed.Assist in team education based on QAPI resultsMonitors processes and work flow across the department and recommends changes for improving efficiency and quality.Provide quality continuum of care visits (Palliative and Hospice – routine, respite and continuous care level of care HospiceMinimum Qualifications
LVN - At least 2 solid years of hospice experience. Palliative certified a plus.Excellent communication and understands and believes the value of hospice and palliative careMust be able to work in ANX Hospice Burlingame office.
Salary : $87,000 - $104,000
