We are seeking a passionate, knowledgeable Pediatric Hospice & Palliative Care Manager that will be responsible for management, oversight, and coordination of the care and services of pediatric hospice and palliative care patients and their families referred to By the Bay Health’s pediatric team, By the Bay Kids.
By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.
Essential Duties & Responsibilities include (but are not limited to) :
- Management of the Pediatric Palliative and Hospice Programs under the direction of the Clinical Director.
- Collaborate with the Director of Patient Care Services to strategically grow the Pediatric program throughout the San Francisco Bay area.
- Participates in development of policies, procedures, protocols, and workflows aligned with anticipated programmatic growth.
- Partners with contracted vendors, hospitals, and facilities to establish strong referral pathways and ensure quality care.
- Receives and / or coordinates new patient referrals and assigns patient to appropriate staff member(s) to assure adequate patient care coverage.
- Schedules, facilitates, and provides leadership to IDT meetings, including facilitation of problem solving, compliance with visit frequency, and delivery of team support.
