As a Behavioral Health Care Coordinator at Apache Behavioral Health Services, Inc., you will play a vital role in providing supportive mental health services to children and adolescents enrolled or eligible for enrollment with our organization.
The Case Manager will provide referral to appropriate behavioral health services and collaborate with assigned clinical teams to ensure comprehensive care. This position will coordinate services between the client, their support system, community, and other healthcare professionals, advocating for the client's needs as necessary.
The key responsibilities of this role include:
- Managing a caseload of clients/potential clients who would benefit from behavioral health services
- Providing support services assistance to ABHS clinical staff, including transportation, community visits, appointment setting/coordination, outreach services for clients having difficulty accessing services, and/or reengagement in treatment services
- Facilitating group counseling, skill training, and education programs in coordination with ABHS clinicians
- Documenting contact with clients to assess needs, maintain required paperwork, and engage family members. Complete assessment updates and treatment plan updates in accordance with Arizona Health Care Cost Containment System (AHCCCS) and ABHS Standards
To be successful in this role, you will need to possess excellent written and oral communication skills, demonstrate strong critical thinking, problem-solving, and organizational and time management skills. A valid Arizona Driver's License and reliable transportation are also required.
