APC Home Health Services is seeking a dynamic and experienced Home Health Director of Nurse to join the leadership team. The Director of Nursing (DON) for Home Health will oversee the clinical operations of the agency's home health program.

Reporting to the CEO, the Home Health DON will be responsible for overseeing and coordinating the day to day clinical operations of the home health service programs and ensure the highest standards of care are maintained. This is a leadership position that demands a deep understanding of nursing practices, regulatory compliance, and a passion for improving the lives of home health and homecare patients. The ideal candidate will possess strong leadership skills and a commitment to patient-centered care, addressing the unique challenges in home health.

Responsibilities:

· Provide clinical oversight and leadership to the nursing and therapy team, ensuring that care is delivered in accordance with established standards and protocols.

· Stay abreast of federal, state, and local regulations and ensure that services adhere to all relevant compliance standards.

· Recruit, train, mentor, and retain a skilled team of nurses and therapists fostering a culture of collaboration, accountability, and professional work.

·Maintain and improve quality assurance initiatives to enhance patient outcomes and satisfaction.

·Oversee the coordination of patient care, including assessments, care plans and evaluation of patient progress.

·Collaborate with interdisciplinary teams to coordinate patient care and achieve organizational goals.

· Advocate for patients and their families, ensuring their needs are met with compassion.

· Partner with the sales team to grow the branch and fully support growth objectives.

· Strong skills in Nursing, home infusion and wound care.

Requirements

• Associate or Bachelor of Science in Nursing
• Current RN license in Texas.
• Minimum of 5 years of nursing experience, with at least 3 years in a leadership role in home health.
• Strong leadership, communication, and interpersonal skills.

Will maintain a high level of communication with CEO and Home care staff.

• Knowledge of regulatory requirements.
