What are the responsibilities and job description for the PHC FIELD SUPERVISOR position at APC Home Health Services, Inc.?
Directly reporting to the designated Office Supervisor, is responsible for administering policies and procedures of the Primary Home Care & Family Care Programs as is set forth in the Texas Department of Aging & Disability Provider Manuals, Managed Care Organizations (HMO’s), AAA, MDCP, etc. by administering and supervising essential job duties of the Attendants hired to perform the Personal Assistance Services for the Clients referred to the agency from DADS and HMO’s. Is responsible for the interviewing , hiring and assignment of competent Attendants to provide Personal Assistance Services limited to: Escort, Home Management, and Personal Care that includes bathing, dressing, grooming, routine hair and skin care, preparing meals, feeding, exercising, and helping with self-administered medications, toileting, and transferring and ambulation services to clients whose health problems cause them to be functionally limited in performing activities of daily living. Must ensure competency of the Attendants through verification of education, experience, or training and will ensure new employment orientation and will provide further on the job training, where indicated. Will secure and submit written, accurate and timely documentation of services provided. Will make scheduled and on site home visits at intervals to evaluate the Personal Assistance Services provided by the Attendant(s) and ensure all agency policies and procedures are adhered to. Will be responsible for assisting other supervisors with the
supervision of their case load in their absence. May be assigned other duties as job merits or when indicated.
