Healthcare Advocate Coordinator

At Apicha Community Health Center, we strive to provide exceptional healthcare services to underserved populations. We are seeking a highly motivated Service Coordinator to join our team in Jackson Heights.

The successful candidate will be responsible for coordinating triage services under the HIV Supportive Services Program, ensuring client eligibility, and facilitating centralized intake, interdisciplinary team case conferencing, and making internal or external referrals as needed.

Key responsibilities include:

• Adhering to AIDS Institute requirements for HIV Supportive Services program
• Conducting intake assessments, screening clients' needs, and working closely with case managers and peer navigators to implement service/care plans
• Developing service plans that incorporate goals, objectives, frequency, and duration of services, including HIV-specific health and nutrition topics
• Coordinating activities with HIV Support Services program staff to ensure adherence to protocols and standards of Department of Support Services
• Providing psychoeducation support and nutrition health education groups to program clients

We are looking for an individual with a strong work ethic, excellent organization, written, verbal, and interpersonal communication skills, and a commitment to our mission and values. Experience working with target populations is preferred, and fluency in Spanish and/or Asian languages is a plus.

Estimated salary: $60,000 - $80,000 per year, depending on experience and qualifications.
