Position: Centralized Admissions Coordinator
Location: Primary location in Avon, CT. Must be willing to travel as needed.
Type:
- Full-Time Monday-Friday
- Per Diem weekends
Are you passionate about ensuring seamless transitions for residents into long-term care facilities? Join our team as a Centralized Admissions Coordinator, where you'll play a pivotal role in guiding individuals and their families through the admission process with care and compassion.
Responsibilities:
- Plan, organize, and execute the admission process, ensuring compliance with all regulations and guidelines.
- Process admissions applications and admit new residents efficiently and effectively.
- Collaborate with nursing, MDS, and rehab departments to facilitate discharge plans for residents.
- Provide guidance and support to existing facility Admissions Coordinators, offering coverage when necessary.
- Offer timely referrals to appropriate agencies for resident/family assistance, such as conservatorship and Medicaid applications.
Qualifications:
- Previous experience in a long-term care facility within Admissions, Social Work, or Nursing required.
- LPN license preferred.
- Knowledgeable of admissions and discharge procedures, current laws, residents’ rights, and documentation requirements.
- Strong attention to detail, follow-through skills, and ability to prioritize tasks.
- Innovative mindset to find creation solutions for resident needs while maintaining a customer focus.
- Possess a vehicle and valid drivers license
- Willing to travel to different facility locations; primary work location at Corporate office in Avon, CT
- Ability to remain calm in stressful situations and communicate professionally with residents, staff, and family members.
