POSITION SUMMARY:

The Intake Coordinator in the Early Intervention setting will manage case coordination for a team of professionals and therapists delivering services to children in the FIT program. This role involves supporting and communicating service progress while providing strategies to help achieve desired outcomes. Additionally, the Intake Coordinator is responsible for assessing the child's ongoing eligibility to continue in the program.

ESSENTIAL JOB FUNCTIONS:

  • Manage referrals, explain the Early Intervention (EI) program, and coordinate the evaluation process for families.
  • Ensure compliance with EI program intake documentation requirements.
  • Conduct face-to-face meetings with families after evaluations, discussing recommended services and next steps.
  • Meet EI program intake billing requirements and deadlines.
  • Serve as the first point of contact for families, providing information about EI services, costs, and parental rights.
  • Schedule the Comprehensive Multidisciplinary Evaluation (CME) during the initial home visit and maintain accurate records.
  • Maintain direct communication with families throughout the intake process to address any concerns.
  • Enter family information into the database and initiate EI records.
  • Collaborate with staff to deliver exceptional, family-focused customer service.
  • Complete and submit accurate documentation to the billing department in accordance with FIT program guidelines.
  • Maintain a reliable vehicle, valid driver’s license, and appropriate auto insurance for home visits.
  • Introduce families to New Mexico’s Early Intervention program and available services following the multidisciplinary evaluation.
  • Contact families within 24 hours of receiving a referral to schedule the home visit.
  • Answer parents’ questions regarding services and costs while documenting required EI information and scheduling evaluations.
  • Request medical records from the child’s primary care physician.
  • Review evaluation reports with families, determine eligibility, and enroll children in the EI program based on their needs.
  • Coordinate intake, evaluations, assessments, and eligibility determination.
  • Facilitate the transition process for each child and their family.
  • Research and provide families with relevant resources and referrals as needed.
  • Meet position benchmarks and performance expectations.
  • Perform other job-related duties as assigned.

REQUIRED/PREFERRED QUALIFICATIONS:

- Bachelor’s degree or higher in the appropriate field of study designated by the FIT program.

- Work experience with case management in a children’s service program is a plus.

- Completion of NM FIT program FSC coursework enabling billing in the EI program.

- Bilingual skills in English/Spanish are required.

- Detail-oriented, organized, efficient, self-starter with strong critical thinking skills and the ability to manage timelines.

- Ability to work in a dynamic environment, gathering relevant information to develop a program for the child and family.

PHYSICAL REQUIREMENTS:

Ability to lift and carry objects weighing up to 40 pounds (e.g., boxes, equipment) without assistance. Capability to stand and walk for extended periods, including moving around the workspace, performing tasks, and interacting with colleagues or customers. Must be able to bend, stoop, crouch, and kneel to access various items, equipment, or workspaces as required. Aptitude to reach overhead, horizontally, and below waist level to retrieve and handle objects or materials. Capacity to perform physically demanding tasks continuously throughout the workday. Physical ability to move freely and quickly in various work environments, including navigating stairs, ramps, or uneven terrain if necessary. If the job involves driving, a valid driver's license, and the ability to operate vehicles safely and responsibly may be required.
