Position Summary
We’re seeking an enthusiastic Intake Coordinator/ Family Services Coordinator to support our mission to help children achieve their highest potential. Positions are available for [full-time in-person, hybrid, or remote] work across New Mexico. The Intake Coordinator /Family Service Coordinator (Intake/FSC) working in the EI setting will perform duties of case management for the team of professionals and therapists who provide services that provide services to the child in the FIT program. The FSC will support and communicate the progress of the services received and offer strategies to achieve the desired outcomes. The FSC determines the child’s continued eligibility to remain in the program

Work Requirements:

  • Managing referral, explains the EI program and manages the evaluation process for the family.
  • Meeting the EI program intake documentation requirements.
  • Providing the face-to-face meeting after the evaluation is complete recommending possible services and communication with the families.
  • Meeting the EI program Intake billing requirements and deadlines.
  • Provides the initial contact with the families after the referral is received explaining EI services, their cost, and the parent’s rights in the EI program.
  • At the first home visit, the coordinator schedules the Comprehensive Multidisciplinary evaluation and maintains accurate details of the meeting.
  • Maintains face-to-face communication during the process to address any family concerns.
  • Enter the family in the database system and begin the EI database records.
  • Work collaboratively with all staff to provide exceptional family focused customer service.
  • Completes and submits quality documents to the billing department as required by the FIT program.
  • A car in good working order, a valid driver’s license, and appropriate auto insurance are required to compete visits in the EI program.
  • The Intake Coordinator position in the EI program introduces the family to the New Mexico Early Intervention program services available after a comprehensive Multidisciplinary to assist their child’s needs.
  • The Intake Coordinator receives the referral from the AIT office staff and contacts the family within 24 hours to set up the home visit appointment.
  • The Intake Coordinator answers questions the parents may have about the services, the cost of these services while documenting the EI required information and setting up the evaluation appointment.
  • The coordinator requests the child’s medical records from the primary care physician.
  • When the Evaluation report is completed, the Intake Coordinator reviews the data with the families and enrolls the child in the EI program according to the child’s needs.
  • Coordinates intake, evaluations and assessments and the process of determining eligibility.
  • Facilitating the development of a transition process for each child and their family.
  • Gathering and researching resource information for the family and making referrals where appropriate.
  • Completes the benchmark required for your position.
  • All other job duties as assigned.

Qualifications & Skills

  • Education: Bachelor’s degree or higher
  • Work experience with case management in a children’s service program is a plus.
  • Completion of NM FIT program FSC coursework enabling billing in the EI program.
  • Bilingual skills English/Spanish are a plus.
  • Detail oriented, organized, efficient, self-starter who with strong critical thinking skills with the ability to handle timelines.
  • Ability to work in a changing environment gathering relevant information to develop a program for the child and family.

Primary Responsibilities

Ability to lift and carry objects weighing up to 40 pounds (e.g., boxes, equipment) without assistance. Capability to stand and walk for extended periods, including moving around the workspace, performing tasks, and interacting with colleagues or customers. Must be able to bend, stoop, crouch, and kneel to access various items, equipment, or workspaces as required. Aptitude to reach overhead, horizontally, and below waist level to retrieve and handle objects or materials. Capacity to perform physically demanding tasks continuously throughout the workday. Physical ability to move freely and quickly in various work environments, including navigating stairs, ramps, or uneven terrain if necessary. If the job involves driving, a valid driver's license, and the ability to operate vehicles safely and responsibly may be required.
