Description
Here at the April Parker Foundation, we are currently seeking a dedicated and compassionate Case Manager to join our team in our Housing program. As a Case Manager, you will play a crucial role in identifying and assisting individuals and families in accessing social support services related to housing. Working closely with our team and community partners, you will be responsible for conducting compassionate outreach to residents who may be facing multiple barriers to care, such as housing insecurity, food insecurity, and transportation issues. Your main focus will be on providing strength-based case management, conducting assessments, advocating for clients, and making community-based referrals and linkages.
In addition to your role as a Case Manager, this position will also require you to work as a Housing Specialist, with the goal of increasing housing opportunities for program participants. You will be responsible for conducting outreach in the community to build relationships with landlords and provide housing search education through counseling and workshops. Your knowledge of the local housing market and understanding of fair housing and landlord / tenant rights will be essential in helping our program participants secure suitable housing.
Requirements
To qualify for this position, we require the following minimum qualifications :
- Bachelor"s degree in psychology, counseling, rehab counseling, or a closely related field
- Minimum of two years of related experience OR Master"s degree in social work, psychology, counseling, vocational rehabilitation, or a closely related field
- Bilingual Spanish (As part of our process to ensure language proficiency, a Spanish assessment is required.)
- Spanish Differential Offered
- Ability to understand the characteristics of a wide range of disabilities
- Knowledge of community resources and services within Orange County
- Participate in team meetings, trainings, local outreach, and professional development activities
- Personal and professional experience with mindfulness or meditation work preferred
- Excellent interpersonal skills and the ability to handle emotionally charged situations
- Effective communication skills with external community partners and clients, both verbally and in written correspondence
- Technical proficiency with Unite Us platform (Training Provided)
