A DAY IN THE LIFE OF SOCIAL SERVICE COORDINATOR
As a Social Service Coordinator at our skilled nursing and rehabilitation facility, you play a vital role in the emotional and social well-being of residents in the facility. You responsible for assessing residents' psychosocial needs, coordinating supportive services, and advocating for residents' rights and preferences. The Social Services Coordinator collaborates with interdisciplinary teams, residents, and families to enhance the overall quality of life for residents.
QUALIFICATIONS FOR A LICENSED PRACTICAL NURSE (LPN)
- Bachelor's or Master's degree in Social Work, Counseling, or a related field.
- Licensure or certification as a Social Worker, if required by the state.
- Previous experience in social services, counseling, or a related role in a healthcare setting, preferably in long-term care.
- Knowledge of psychosocial assessments, care planning, and discharge planning processes.
- Strong communication and interpersonal skills.
- Empathy and a compassionate approach to resident care.
- Ability to work collaboratively in a multidisciplinary team.
- Familiarity with state and federal regulations related to social services in healthcare.
