Summary:
The Intake Coordinator will be the primary person responsible for residential intake for
Genesee, Livingston, Orleans, and Wyoming counties, and will also support the intake and process for Day, Vocational, and Community Services for Genesee, Livingston, Orleans, and Wyoming Counties.

Ability to use standard office equipment including personal computer, phone, fax, copier, etc. Ability to drive a vehicle and sit or stand for extended periods of time. May be required to walk up and down stairs and lift up to 40 pounds.
Working Conditions:
Working conditions are normal for an office environment with the potential to travel amongst Genesee, Livingston, Orleans, or Wyoming counties.

Essential Functions:
- Incorporate the agency’s mission and vision in the performance of all job duties.
- Maintain strong working relationships with internal program staff, Care Coordinators, DDRO staff (from Finger Lakes and Western Regions), and other referring entities
- Provide a caring and respectful atmosphere to those seeking services
- Strong and positive communication skills with internal and external entities
- Assist in developing Standing Operating Procedures for handling Intake within the agency’s Electronic Health Record
- Successfully complete all mandated agency trainings
- Adhere to agency policies and procedures
- Perform other related duties as assigned by the CEO and/or designee
- Participate in community-based events, including events with Public Relations when requested, to promote the agency’s supports and services.
- Serve on committees as request
- Participates in corporate compliance and quality assurance duties as required.
- Provide agency training as needed
- Identify best practices related to the intake/admission process, and work on creating a unified system/process for Residential Intake in our four counties
- Work in partnership with Finger Lakes and Western DDRO’s on screening applicants for GLOW’s residential openings.
- Work with Care Coordinators and other referring entities to ensure we have necessary documentation for program consideration
- Work in collaboration with respective program supervisors, Care Coordinators, and individuals and families to arrange for tours/visits as necessary
- Facilitate and participate in the Residential Admission Team (RAT) meetings to share information about individuals seeking services
- Help guide the admission process for individuals, and help keep the process moving forward by providing reminders and prompting as needed
- Participate in monthly vacancy management meetings with the DDRO
- Ensure completion of required paperwork for the respective DDRO pertaining to admissions and discharges (i.e. Vacancy Announcements, Request to Fill, Internal Move Notification, Residential Screening Report, etc)
- Help maintain (informal) waitlist for future consideration
- Document and Track activity to maintain awareness of where individuals are in the referral process
- Track/trend screenings, tours/visits, and admissions
- Work with Care Coordinators and other referring entities to ensure we have necessary documentation for program consideration
- Intake processes will include various tasks such as mailings, e-mails, tracking data, and saving/uploading information to electronic health record as appropriate, amongst other tasks.
- Work in collaboration with respective program supervisors, Care Coordinators, and individuals and families to arrange for tours/visits as necessary (when deemed appropriate)
- Help maintain waitlist where applicable

- Strong analytical skills beneficial
- Strong organizational skills
- Strong computer and database skills
- Knowledge and understanding of various eligibility programs, community resources and ability to navigate the system.
- Strong interpersonal skills
- Use of personal vehicle is required
- A minimum of a Bachelor’s Degree in Health or Human Services and a minimum of 1 year experience in working with people with I/DD
- OR an Associate’s Degree in Health or Human Services and a minimum of 5 years working with people with I/DD.
- A valid NYS Driver’s License and safe driving record, per Agency policy.
- Strong working knowledge of the agency’s programs and services
