The MDS Coordinator, who reports to the Director of Nursing Services, leads the Interdisciplinary Team (IDT) to ensure MDS assessments are completed in compliance with State and Federal regulations.
Key Responsibilities:
- Oversee the Medicare/MDS resident assessment process.
- Ensure timely completion of MDS assessments by the Interdisciplinary Team.
- Manage the creation, implementation, and evaluation of care plans.
- Administer prescribed treatments, medications, and nursing interventions.
- Provide essential nursing care while adhering to infection control protocols.
- Follow safety guidelines during nursing procedures.
- Execute emergency protocols according to center policy and nursing standards.
- Facilitate the exchange of vital information for optimal resident care.
- Maintain all necessary documentation in accordance with Federal and state regulations, as well as Company policies.
- Participate in all required meetings and training sessions.
- Exhibit professionalism in attire, conduct, and communication with clients.
- Follow Company standards when addressing client inquiries.
- A valid LVN license is required.
This is a full-time position, Monday through Friday, with occasional weekend shifts.
Salary : $40 - $45
