REQUIREMENTS:
- Diploma from an accredited Nursing school
- Valid RN/LVN license in hand
- Bonus points for 1-2 years of long-term care experience
- Communication skills that dazzle, both written and spoken

REPORTING: The MDS Nurse answers to the Director of Nursing Services.
YOUR MISSION, SHOULD YOU CHOOSE TO ACCEPT IT:
- Master those MDS Assessments for HMO, Medi-Cal, and private folks per OBRA rules.
- Spot the triggers and wrap up the R.A.P. summary.
- Kickstart the resident care plan based on R.A.P., diagnosis, and doc orders.
- Nail those quarterly assessments.
- Keep the care plan on point by following up on assessments.
- Update care plans like clockwork every quarter.
- Track discharges for HMO, Medi-Cal, and private residents per OBRA rules.
- Juggle MDS Assessment dates with the team and maintain the log as directed.
- Schedule Care Conferences and loop in the team two weeks ahead.
- Lead Care Conference meetings with flair, ensuring residents are the stars.
- Be part of the all-star Interdisciplinary Team as the Director instructs.
- Follow Facility Policies and Procedures. Attend in-service meetings as needed.
- Be the role model everyone looks up to.
- Create a positive, zen workplace.
- Dive into direct patient care when needed, as per the Director's guidance.
- Join committees as assigned by the Director.
- Work hours approved by the Director and/or designee.
- Embrace other tasks from the Director or Administrator as your next adventure!
