Social Services Coordinator - Archbold Living Thomasville
Position Description
Demonstrates effective organizational, communication and leadership skills.
In the absence of an Activity Director, ensures that all activities are carried out and the residents' needs are met.
Coordinates new admissions for facility.
Assures an ongoing social services program to meet the need of the residents.
Evaluate social and family information and assist in determining plans for social treatment.
Plan, coordinate and notify family members for social and activity quarterly family night meetings.
Hand out mail, read and write letters for residents when requested.
Shops and makes purchases of personal items needed by residents.
Upon admission, orients residents and families on residents' rights and advanced directives.
Keep an adequate filing system for social services data.
Keep families aware of residents' condition and update if they are sick or in hospital.
Coordinate social service activities with other departments as necessary.
Interpret social, psychological and emotional needs of resident and family to the medical staff, their doctor and of the resident's response to the service.
Be alert for any change in resident condition, report change to charge nurse immediately.
Review and revise care plan and assessments as necessary, but at least quarterly.
Assist in developing plan of care for each resident that identifies the problem/needs of the resident and goals to be met.
Assist in the scheduling of care plan meetings (weekly) insofar as practical, encourage resident participation.
Visit residents on a daily basis to assure social/activity needs are being met.
Maintains a working relationship with other long-term care facilities in the area.
Assures the rights of residents are protected.
Ensure all employees and volunteers are knowledgeable of the residents' rights and responsibilities.
Ensure all residents are treated fairly, with kindness, dignity and respect.
Promote maintenance or enhancement of the quality of life for each resident.
Experience/Education
High school graduate or GED equivalent preferred
Minimum of two years experience working in a skilled nursing care facility or other related medical facility.
Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing social programs.
Must be willing to complete the thirty-six hour workshop for GSNHSW sponsored by the Georgia Nursing Home Association.
Must be able to lift a minimum of fifty pounds, able to push, pull and move supplies.
Must be able to function independently, have flexibility, personal integrity and to work effectively with residents, personnel and support agencies.
