We are seeking a dedicated and organized individual to join our team as a Hospital Liaison/ Home Health Intake Coordinator. The successful candidate will have experience collaborating with physicians, specialists, and other medical professionals. The Hospital Liaison/ Home Health Intake Coordinator is responsible for managing all aspects of the patient intake process, establishing and maintaining positive relationships with patients and referral sources, responding to patient requests and concerns, and managing the insurance verification and authorization processes.
HOSPITAL LIAISON/ HOME HEALTH INTAKE COORDINATOR JOB FUNCTIONS/RESPONSIBILITIES
- Executes all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.
- Establishes and maintains positive working relationships with current and potential referral and payer sources.
- Assists members of the marketing team as needed.
- Provides assistance in strategic planning including identifying opportunities for additional or improved services to address customer needs.
- Maintains comprehensive working knowledge of Archer Health markets including government agencies, major payer groups, and key referral sources.
- Maintains comprehensive working knowledge in the field of intake, marketing and shares information with appropriate organization personnel.
- Maintains comprehensive working knowledge of community resources and assists customers in accessing community resources should services not be provided by Archer Health.
- Maintains comprehensive working knowledge of Archer Health, Inc contractual relationships and ensures that patients are admitted according to contract provisions.
- Gathers, collates, and reports referral statistics including key customer referral trends.
- Builds and monitors community and customer perceptions of Archer Health, Inc as a high quality provider of services.
