Assistant to Behavioral Health
ACHC is a Federally Qualified Health Center and licensed primary care clinic. We provide medical and dental care with additional specialists in Chiropractic, Internal Medicine, Neurology, Gynecology, Pediatrics, Psychology, Podiatry and Optometry. ACHC Clinics are located across Fresno, Kings and Tulare counties.
SUMMARY
The Assistant to Behavioral Health is responsible for screening all eligible patients using behavioral health screening tools to identify potential behavioral health conditions. Facilitate referrals internally and externally to appropriate behavioral health services as warranted and work closely with in-house behavioral health therapist on scheduling, maintaining caseload and patient coordination of care.
RESPONSIBILITES

- Exercise professional judgment when following ACHC procedures, protocols, and position requirements and in the absence of ACHC procedures, protocols, and position requirements.
- Use established communication protocols within ACHC, across agencies, health systems and in the community.

- Accept accountability for own actions and decisions.
DUTIES
- Provide patient-centered care through interacting, planning, and coordinating on a multi-disciplinary team.
- Liaise with social workers, clinicians, case managers, and other healthcare providers.
- Provide assistance with paperwork, file organization, data entry, and other administrative tasks as required.
- Assists with patient check-in/check-out, appointment reminders, missed appointment follow up, managing provider schedules, and patient scheduling/rescheduling, as needed.
- Works with mental health care providers to ensure timely delivery of risk assessment, screenings/questionnaires, intake, diagnosis, treatment and wellness/maintenance follow up.
- Maintain clear, concise, accurate and timely documentation of patient encounters.
- Provide linkages and referrals to community resources and assistance with navigating service delivery networks to find required resources.
- Assist in facilitating transitions of care, addressing care gaps, conducting high-risk patient follow-ups, and coordinating referrals.
- Participate in Behavioral Health integration initiatives and other service models aligned with patient-centered, evidence-based models of care.
- Provide education and consultation regarding behavioral health issues to community-based service providers and patients.
- Development and implementation of outreach efforts to facilitate patient engagement in behavioral health services.
- Facilitate individual/group educational workshops/sessions, as needed.
- Drive between sites as needed.
- Organize workload effectively (e.g. time management, prioritizing, setting time frames, delegation).
- Other duties as assigned.
Qualifications
QUALIFICATIONS
- Analytical skills in order to: elicit and interpret information from patients, families, community health service providers and other sources. initiate and collaborate with an interdisciplinary team.
- Verbal and written communication skills to work effectively within an interdisciplinary team, including patients, families/significant others.
- Interpersonal skills to work effectively with interdisciplinary team members and establish relationships with patients.
- Ability to use computer software programs (Word, Excel, etc.).
- Excellent verbal and written use of the English language.
- Bilingual Spanish required.
- Experience with Electronic Health Records preferred.
EDUCATION AND EXPERIENCE
- High school diploma required
- Some College in behavioral health, social sciences of other related field preferred
- Two years of experience in healthcare or related field preferred
- Experience with behavioral health terminology preferred
PHYSICAL/COGNITIVE REQUIREMENTS
- Hearing: Adequate to perform job duties in person and over the telephone.
- Speaking: Must be able to communicate clearly with patients in person and over the telephone.
- Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.
- Cognitive: Aptitude to complete tasks, including abilities such as learning, remembering, focusing, categorizing, and integrating information for decision making, problem-solving, and comprehending.
- Other: Requires occasional lifting and carrying items weighing up to 20 pounds unassisted. Requires frequent bending, reaching, and repetitive hand movements (specifically keyboarding and writing), standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular work shift.
