This is a full-time on-site role as a Targeted Case Manager at ARIA Kentucky at our Paducah, Ky location. The Targeted Case Manager will be responsible for providing individualized support and assistance to clients, conducting assessments, developing treatment plans, coordinating services, and monitoring progress towards recovery.
