POSITION TITLE: Full Time Rehabilitation Specialist/PRP Supervisor
REPORTS TO: Clinical Director
DEPARTMENT: Psychiatric Rehabilitation Program (PRP)
Job Description:
Responsible for organizing program services and support to ensure that clients needs are being met through strengthening their decision-making skills, personal and professional growth and navigating resources in the community through the assistance of their Rehabilitation Coordinator
Requirement: Licensed mental health professional with a minimum of 2 years direct care experience working with youth and adults with a mental health diagnosis.
Specific Duties:
Responsibilities and Duties
- Complete initial screenings for all individuals referred to the program.
- Complete client assessments for all individuals entering the program.
- Review diagnostic impressions and ensure validity of diagnosis with client presentation, collaborating with referring therapist and documenting if changes have been observed in client’s clinical presentation.
- Ensure that each client has a current and comprehensive individualized rehabilitation plan.
- Ensure that all client contact is documented appropriately in the form of session notes.
- Ensure that each client has a current and comprehensive rehabilitation monthly progress report
- Oversee the day to day operations of the PRP program
- Facilitate onboarding and termination of PRP staff
- Conduct initial assessments and develop Individual Rehabilitation Plans
- Conduct on-going assessment of staff’s performance
- Educate Rehab Coordinators on the correlation between rehabilitation activities and the goals identified in the IRP
- Ensure all required documentation is completed in a timely, comprehensive and legible manner
- Facilitate training for PRP staff
- Navigate community resources and support to promote community integration
- Provide supervision based on staff supervision plan which may be weekly, biweekly or monthly
- Obtain and maintain client authorizations (Reauthorizations should be completed 30 days before expiration of authorization)
- Assign caseloads to PRP staff
- Coordinate monthly group events/activities
- Participate in professional development courses/workshops/trainings (CEU)
- Participate in multi-disciplinary meetings
- Promote and marker PRP program to establish community partnerships and referrals
- Provide quality assurance calls to clients and/or support system
- Provide productivity reports to Clinical Director
Experience:
- At least 2 years with experience working with youth and adults with a mental health diagnosis
Education:
- Master's (Required)/ CPRP
Maryland License:
- LGPC, LMSW, LCPC, LCSW-C or CPRP (Required)
