Hospice Liaison

Overview
We are seeking a motivated and dynamic Liaison to serve as a critical link between our organization and patients. We are looking for someone who can facilitate communication between a hospice agency and other healthcare providers, primarily by assessing potential patients for hospice eligibility, discussing care options with patients and families, and coordinating the admission process to ensure smooth transition into hospice care; essentially acting as a bridge between the hospice and the referring facility or community to facilitate patient intake and ensure appropriate care delivery.

Duties

• Check in with facilities
• Patient and family education
• Help with patient care planning
• Patient assessment and eligibility review
• Participate in community events and presentations about hospice services.
• Maintain accurate patient records and submit required documentation to relevant parties.

Qualifications

• Proven experience in a liaison role or similar position, demonstrating strong leadership skills.
• Exceptional customer service abilities with a focus on building lasting relationships.
• Strong analytical skills with the ability to assess situations and provide effective solutions.
• Understanding of end-of-life care, disease progression and symptom management.
• Empathy and ability to provide emotional support to patients and families facing end-of-life decisions.
• Managing a caseload efficiently and maintaining accurate documentation.

Job Type: Full-time

Pay: $50,000.00 - $60,000.00 per year
