Job Title: Case Manager 
Work Site: Vienna, IL 
Area of Focus: Behavioral Health
Program(s): Supervised Residential Program (830) provides adults living with substantial levels of mental illness or disability with 24/7 services through recovery-oriented community residential facilities.
Immediate Supervisor: Sr. Program Director 
Hours of Work: Full-time, 40 hours/week
Minimum Qualifications: High school diploma or GED equivalent; Qualifies as a Mental Health Professional (MHP) within 12 - 18 months of hire.
Skills & Knowledge: Must have knowledge of case management processes, area resources/service systems, word processing and basic computer skills. Employee must be able to assist supervisor in addressing individual client treatment needs and document clients’ progress toward the goals and objectives identified on the Individual Treatment Plan. Excellent written and oral communications, ability to teach basic life skills and problem solving strategies are required.
Special Requirements: Must be 21 years of age, maintains a valid driver’s license and vehicle liability insurance, maintain First Aid & CPR certification, must complete required registry clearances and criminal fingerprint background checks, travels to all agency sites when appropriate, schedules evening or weekend hours as necessary, after-hours crisis response rotation if MHP eligible
Job duties include, but are not limited to:
  • Provides case management and community support services throughout the clients’ stage of treatment with a focus on engagement, health care coordination, and discharge planning.
  • Intervenes to change attitudes, beliefs and behaviors of clients to improve individual and family level of functioning.
  • Manages transportation for resident medical appointments as needed.
  • Facilitates transportation services to assist clients in connecting with internal and external resources.
  • Maintains networking and referral contacts with other human service providers both internally and externally while assessing barriers for successful engagement, treatment, and discharge.
  • Completes required monthly direct service hours to clients.
  • Facilitates recreation/socialization and cultural activities with clients.
  • Facilitates linkage and referral to appropriate resources and opportunities as identified in the client’s treatment plan.
  • Completes all required forms and documentation in a timely and efficient manner.

Salary Range: $32,932 - $45,133 annually
*This is a bargaining unit, union position
