- Provides case management and community support services throughout the clients’ stage of treatment with a focus on engagement, health care coordination, and discharge planning.
- Intervenes to change attitudes, beliefs and behaviors of clients to improve individual and family level of functioning.
- Manages transportation for resident medical appointments as needed.
- Facilitates transportation services to assist clients in connecting with internal and external resources.
- Maintains networking and referral contacts with other human service providers both internally and externally while assessing barriers for successful engagement, treatment, and discharge.
- Completes required monthly direct service hours to clients.
- Facilitates recreation/socialization and cultural activities with clients.
- Facilitates linkage and referral to appropriate resources and opportunities as identified in the client’s treatment plan.
- Completes all required forms and documentation in a timely and efficient manner.
