Job Title: Community Support Peer

Home Office: Vienna, IL; Anna, IL; Golconda, IL; or Cairo, IL. Location is dependent on availability and proximity to home residence.

Area of Focus: Behavioral Health

Program(s):

  • Behavioral Health: Outpatient Mental Health Programming helps individuals build life skills and well-being through behavioral health counseling, psychiatric care, and other care coordination and support services.
  • SAMHSA CCBHC: Serves individuals living with a mental or substance use disorder who seeks care, including those with severe mental illness (SMI), substance use disorder (SUD) including opioid use, children and adolescents with serious emotional disturbance (SED), individuals with co-occurring mental and substance disorders (COD), and individuals experiencing a mental health or substance-related crisis.

Immediate Supervisor: Programs Coordinator

Hours of Work: Full-time, 40 hours/week

Community Support Peer Minimum Qualifications: High School Diploma or GED; qualifies as a Mental Health Professional (MHP) within 12 – 18 months of hire; must be at least 21 years of age.

Community Support Peer Skills & Knowledge: Skills in the delivery of case management and support services; knowledge of local health and human services resources; excellent written and oral communication skills; basic computer skills; ability to navigate enhanced phone system.

Community Support Peer Special Requirements: Has lived experience in areas of mental health, substance use, or navigating crisis systems; resident of Alexander County, Pulaski County, Johnson County, Union County, Massac County, Pope County or Hardin County preferred; ability to work evening and weekend shifts, as applicable; must obtain the Certified Recovery Support Specialist (CRSS) credential within 1 year of hire; maintain First Aid & CPR certification; must complete required registry clearances and criminal fingerprint background checks; travel to all agency sites when appropriate; use of personal vehicle when agency vehicle not available; must have a valid driver’s license and proof of vehicle liability insurance; participates in after hours and day call crisis rotation for peer response call.

Community Support Peer Job duties include, but are not limited to:

  • Provide community-based support to build on independent activities of daily living such as grocery shopping, attending medical appointments, assisting with medication reminders, social skills training and development, money management and budgeting skills, and coping skills development.
  • Facilitates communication externally and across agency staff/applicable programs to ensure continuity of care for individuals presenting in crisis services.
  • Provides comprehensive consumer care and case management services, onsite and offsite as identified by client needs, including access to transportation services, state and federal entitlements, medical care, psychiatric care, housing, and other basic needs to support stabilization and reduce re-entry into crisis.
  • Teaches life-management and daily living skills to clients that translate to their natural environment with an emphasis on developing natural supports.
  • Conducts outreach to individuals who are in or at-risk of entering crisis systems of care in an effort to assess for access barriers and engage them into treatment and other stabilization supports. This includes determining if individuals meet criteria for Community Support Team or other services within the region that may be more applicable.
  • Maintains networking and referral contacts with other human service providers both internally and externally.
  • Completes all required forms and documentation in a timely and efficient manner.
  • Demonstrates active involvement in team/program meetings, supervision, and agency/community events and activities as assigned.
  • Records and documents consumer treatment by following established policies and procedures.
  • Operates as the initial access point for individuals calling the Agency’s 24/7 resource and crisis line.
  • Assesses for presenting needs and the immediate safety of individuals in crisis; facilitates warm handoff to Crisis QMHPs and/or first responders, as applicable.
  • Provides phone-based peer support to individuals who do not present as an imminent safety risk that are not in need of a face to face response.
  • Fields inquiries from Illinois’ Call4Calm line to assess immediate needs and link to identified resources.
  • Provides of-site peer support and stabilization services during crisis intervention when deemed necessary by QMHP/Crisis Director.
  • This is a bargaining unit, union position
