Job Title: Resident Wellness Coordinator
Home Office: Autumn Ridge (AR)
Area of Focus: Community Collaboration
Program(s): Supportive Living Program (SLP) helps adults age 65 live independently by providing them with housing in an apartment-style setting that emphasizes privacy and autonomy, while also offering services that support their health and well-being and providing a little extra assistance with the activities of daily life.
Immediate Supervisor: Resident Wellness Director - Jacinta Hupe, RN
Hours of Work: Full-time 40 hours per week.
Resident Wellness Coordinator Minimum Qualifications: GED/HS Diploma required; maintain BLS Certification; Illinois LPN license required; supervisory experience preferred; valid driver's license; minimum liability automotive insurance.
Resident Wellness Coordinator Skills & Knowledge: Basic understanding of or ability to learn infection control and regulations for workplace safety. Implement resident specific objectives as outlined in individual service plans and assists in performing activities of daily living through which objectives may be achieved or maintained.
Resident Wellness Coordinator Special Requirements: Evening/Weekend availability is required; must be able to lift a minimum of 50 Lbs. unassisted; should be able to lift 195 Lbs. with peer assistance or mechanical (hoyer lift); must be 21 years or older to drive agency vehicles; prolonged periods of sitting is required; prolonged periods of working on a computer; travel to all agency sites when appropriate; must complete and maintain the required registry clearance and criminal fingerprint background checks; participates in On Call Rotation
Resident Wellness Coordinator job duties include, but not limited to
- Provides direct supervision to Support Technicians, including job applicant interviewing, hiring recommendations, employee orientation, performance evaluation, administration of discipline, and recommendations for unpaid suspension or termination, revision of job descriptions and day-to-day work assignments and medical staff schedule.
- Address residents’ specific nursing goals as outlined in the resident’s individual care plan.
- Ensures that Supportive Living programs maintain compliance with all applicable standards and regulations and will ensure that compliance occurs with given policies / procedures on a day to day basis.
- Participates and instructs the residents in recreation and leisure activities that will encourage community integration and inclusion, as needed or available.
- Documenting incident reports on the service summaries/plans, notifying MCOs of resident incidents and forwarding the incident reports to the safety manager.
- Provides or assists with transportation for residents when required as an integral part of the service care plan, as needed or available. Transfers residents safely in/out of the vehicles and safely/correctly operates the vehicles adaptive equipment.
- Responsible for administering injections, TB skin test and other medications, when nurse administration is necessary.
- Assists with nursing information and monitors and documents the health status of each resident.
- Assist with compiling data for resident assessment instrument, Individual Service Summaries and individual service plans for residents to maintain compliance per HFS guidelines. Updating new orders to the service summaries/plans.
- Provides direct services in Supportive Living programs as needed to temporarily meet increased demand for services, when staff ratios must be maintained or for regulatory compliance.
