ACT (Assertive Community Treatment) Specialist

Reporting to the ACT Team Leader, the ACT Specialist is primarily responsible for aiding AOR clients in achieving mental health goals outlined in treatment plans. Delivering 75% of care in natural settings (home, community), they assess personal, medical, emotional, and environmental safety during visits. ACT Specialists possess the capacity to make decisive interventions, especially in crises and health evaluations.
All ACT team members must wholeheartedly embrace AOR’s mission, delivering person-centered, strengths-based, trauma-informed care to clients with mental illness and co-occurring substance use disorders, utilizing a recovery and harm reduction approach.
An ACT Specialist’s primary duties include: provide direct service to clients within a collaborative caseload, complete and juggle various case management tasks, document all services daily, and provide crisis/on-call coverage, as needed.
Responsibilities
- Conduct well-being assessments via home, community, and telehealth visits.
- Complete daily service documentation and required paperwork in the Electronic Client Record (ECR), including IM CANS, LOCUS, and other forms.
- Integrate client feedback on treatment goals quarterly, involving the client's support system and care providers.
- Identify and liaise with upline for necessary care level adjustments or referrals.
- Teach clients new coping skills for stabilization during mental or emotional challenges.
- Provide ongoing psychoeducation, medication training, and monitoring, coordinating with the ACT RN Specialist for medication accuracy and documentation.
- Assist clients in enhancing independent living and daily life skills (IADLs and ADLs).
- Support entitlement applications (SSI/SSDI, Medicaid, SNAP).
- Overcome transportation barriers by providing rides using personal vehicle when needed.
- Model evidence-based practices: client-centered, recovery-oriented, trauma-informed, and harm reduction techniques.
- Engage actively in daily communication with upline, teammates, and providers.
- Participate in team meetings, sharing updates to enhance the collaborative model.
- Offer on-call and crisis support, potentially including weekends for the 7-day ACT model.
- Coordinate care across disciplines and with family members to ensure seamless client support, including psychiatry.
- Achieve direct service targets and ensure accurate and timely service documentation.
- Fulfill assigned related duties and projects.
Qualifications
Must meet one the following requirements:
- Rule 132 credential requirements for a Licensed Practitioner of Healing Arts (LPHA) or a Qualified Mental Health Professional (QMHP), including a master’s degree in social work, counseling, psychology, human services, drug & alcohol, education, or criminal justice
- Or a bachelor’s degree in human behavioral science which includes 30 semester or 45 quarter hours either in development of human behavior, child development, family intervention techniques, diagnostic measures, or therapeutic techniques, such as social work, psychology, sociology, guidance and counseling, and child development.
- 0-2 years minimum of experience working with persons diagnosed with mental illness
- Valid Illinois Driver’s License with daily access to a vehicle with $100,000/$300,000 liability insurance
