Position Summary
The Nurse Navigator coordinates diagnostic and treatment processes and follows up at assigned Asante facility(s). Guides patients and their families through the continuum of care and collaborative process with medical care providers and ancillary support programs. Acts as a central point of contact for all medical care providers working with the patient, and facilitates evidence based nursing care. Assesses educational needs and designs, teaches and/or coordinates educational programs for patients, families and the community.
Qualifications
Education
- Bachelor of Science in Nursing, or equivalent combination of education and experience, required
- Master’s Degree in Nursing, preferred
Experience
- 3 years of clinical nursing experience, including bedside patient care; required.
Licensure/Certifications
- RN: Registered Nurse licensed by Oregon State Board of Nursing is required upon start
- BLS: Basic Life Support for the Healthcare Provider certified by the American Heart Association (AHA) is required upon start
- RNC-OB: Registered Nurse Certified-Inpatient Obstetrics Nurse, preferred
- RNC-MNN: Registered Nurse Certified-Maternal Newborn Nurse, preferred
- C-EFM: Electronic Fetal Monitoring Certified, preferred
