Program Position : Behavioral Health Specialist
Reports to : Director of Maternal Child Health and Program Manager
Job Specification : 40 hours / week. This is a salary, exempt position, that is grant-funded.
Salary : $81,120
Summary :
Care Ring - a fast-growing and highly respected community organization seeks a passionate leader for the role of a Behavioral Health Specialist for the A Guided Journey-Healthy Start Enhanced (AGJ-HSE) Program. The goal of this role is to address behavioral health and substance use of pregnant and interconceptional clients by assessing needs, making referrals to appropriate behavioral health resources and specialists, and providing short-term counseling and evidence-based treatments as indicated. The Behavioral Health Specialist coordinates and supports behavioral healthcare for our patients, in collaboration with the Medical Director and Case Management team.
Responsibilities : Communication :
- Manages daily schedule in Simple Practice following program protocols for patient visits including telehealth
- Participates in meetings that address patient progress and barriers to patient care
- Represent Care Ring and its priorities to individuals, the public, and other organizations at the local, state, and national level
Administrative :
Supports policies, procedures, guidelines, and standards of Care RingAdminister behavioral health assessment process for Healthy Start Enhance ProgramWorks cooperatively and collaboratively as a member of a Care Team with primary care providers, MAs, and other clinical staff to provide care to patients based on best practices and evidence-based medicine (EBM)Documents in the Electronic Medical Record (EMR) and verbally communicates with the Care Team to coordinate interventions and facilitate continuity of careStay in touch via tasks, email, routing notes, and in-person supervision as well as access immediate supervisory support to discuss safety or crisis concerns when indicatedAttend weekly AGJ staff meetingsClinical :
Provides crisis intervention, evidence-based brief interventions such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing, and self-management support for referred patientsProvides strategic and goal-oriented counseling working with patients in a short-term modelConducts telehealth or home visits to clients enrolled in the Maternal Child Health (MCH) programDevelop positive working relationships with program participants and their families or representativesFacilitates patient and family education, and promotes continuity of care to achieve optimal patient outcomesAssures patient rights by offering a choice when appropriateMonitors the condition of patients and reviews the patient plan of care with the multi-disciplinary teamAbility to provide targeted, solution-focused interventions with patients to improve their emotional and physical health.Demonstrates knowledge of DSM (Diagnostic and Statistical Manual of Mental Disorders) and the ability to perform thorough social and mental assessmentsEvaluate a wide range of psychosocial impairments within a patient population from adolescents to older adults and / or arrange for more detailed assessments when indicatedProvides well-conceived, best-practice recommendations when the plan of care is not sufficientDemonstrate sensitivity to vulnerable populations and ability to provide services that are relevant to the culture of the consumer and their familyQualifications and Education Requirements :
Education : Current behavioral health or counseling licensing is required. CADC (Certified Alcohol and Drug counselor) preferred; CPR requiredVaccination : Proof of Covid-19 vaccination requiredExperience : One year (1) of experience in a community or medical environmentSkills :Knowledge of and experience with substance abuse, addiction, and recovery issues
Ability to function without direct supervision, utilizing time constructively and organizing assignments for maximum productivityAbility to develop rapport with and provide emotional support for patients and / or families and staff in crisis situationsAbility to establish relationships and coordinate services between health care and community service providers and the patient and / or family and staffExperience with assessment and treatment planning for common mental health and / or substance use disordersFamiliarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation)Strong interpersonal and listening skills required to communicate effectively in both written and oral forms, including presentationsAbility to work in a multi-cultural environment with diverse groups of peopleMust be proficient with business software including Microsoft Office (Word, Excel, Outlook, PowerPoint, etc.)Bilingual in English / Spanish is a plusValid driver's license and car insurance required.Position Requirements :
