Program Position : Community Health Worker
Reports to : Team Lead and Program Manager
Job specification : A Guided Journey (1.0 FTE). This is an hourly and non-exempt position, that is a Grant funded with an end date of 06 / 30 / 25
Salary : $19.40 / hour
Schedule : Monday-Friday, 8am-5pm with occasional evening and weekends
Summary :
Care Ring - a fast-growing and highly respected community organization seeks a passionate individual for the role of Community Health Worker (CHW) for A Guided Journey program. The CHW provides case management, coordination of care and health education to A Guided Journey program participants. As a trusted member of the community, the CHW will facilitate access to services and work to provide high quality and culturally competent service delivery. The CHW must have a passion for Maternal-Child Health to support improved birth outcomes, and family health well-being. The goal of this role is to increase perinatal health knowledge and self-sufficiency among our participants. We do this through a range of activities such as outreach, community education, informal counseling, social support, and advocacy.
Basic Job Functions
- Identify and enroll eligible participants based on A Guided Journey qualifying criteria.
- Provides home and community-based care coordination and linkage to community resources to program participants and their families.
- Provide evidence-based and evidence-informed education including, but not limited to : breastfeeding, immunization, safe sleep, family planning, smoking cessation, nutrition, mental health, physical health, stress, domestic violence.
- Screen for depression and refer to community agencies as necessary.
- Demonstrates open and respectful communication in all interactions with program participants and their families
- Maintains accurate records of program participants and submits information in a timely manner to meet grant productivity requirements.
- Maintain HIPAA and confidentiality
- Complete all training and ongoing professional development for CHW certification
- Attend meetings, events and conferences
- Follows policies, procedures and best practices of Care Ring and the A Guided Journey model.
- Participates in community outreach activities.
- Promotes public awareness of Care Ring and A Guided Journey model.
- Reports suspected cases of child and / or adult abuse / neglect / exploitation. Serves as liaison during evaluation and investigation.
- Conduct home visits in all areas of Mecklenburg County
