Summary :

The Community Based Service Manager leads an interdisciplinary team dedicated to addressing social determinants of health (SDOH) and healthcare access barriers. Through strategic community partnerships, program management, and team leadership, this role bridges Care Ring's services with community needs, creating meaningful health impacts for underserved populations across multiple locations, including the North End and Grier Heights communities. The ideal candidate is passionate about wellness, skilled in collaboration, program management, staff supervision, and experienced in executing programs that create meaningful impacts.

Responsibilities :

  • Lead an interdisciplinary community-based team, fostering a culture of collaboration and continuous improvement
  • Manage The Bridge program, coordinating mobile unit operations and community-based care delivery
  • Serve as the United Way United Neighborhoods liaison for North End and Grier Heights operations, overseeing Care Ring's centrally located food pantry and coordinating neighborhood-based initiatives
  • Provide strategic oversight of food pantry operations, ensuring alignment with organizational goals and SDOH initiatives
  • Support the Food Pantry Coordinator in developing volunteer management strategies, training programs, and operational procedures
  • Develop and implement SDOH screening protocols, referral pathways, and follow-up processes
  • Set and monitor team performance metrics, conduct evaluations, and support professional development
  • Cultivate and maintain strategic partnerships with healthcare providers, social service agencies, and food suppliers
  • Create comprehensive referral networks to addressing SDOH needs (housing, food insecurity, transportation)
  • Represent Care Ring at community forums, developing opportunities for program expansion and resource integration
  • Design and implement outreach strategies, including educational workshops and communication materials
  • Monitor and evaluate program effectiveness using qualitative and quantitative data
  • Prepare regular performance reports for leadership and outcomes and impact
  • Ensure alignment of all activities with organizational goals and SDOH initiatives

Qualifications and Education Requirements

  • Bachelor's degree in Public Health, Social Work, Communications, or a related field
  • Minimum of 2 years of experience in community engagement, outreach, or program coordination
  • At least 2 years of supervisory experience, preferably in a nonprofit or community-based setting
  • Knowledge of barriers to health care for the low-income and / or uninsured population
  • Proficiency in Microsoft Office, including Word, Excel, PowerPoint and Adobe PDF
  • Strong interpersonal, organizational, and project management skills
  • Experience working with a diverse population preferred
  • Ability to travel to outreach and enrollment events, must have reliable transportation
  • Must have a valid driver's license and proper car insurance.
  • Preferred Skills

  • Knowledge of healthcare systems, medical terminology, and NC community resources
  • Deep understanding of health disparities and SDOH impacts on vulnerable populations
  • Proven track record of building effective community partnerships and referral networks
  • Strong oral and written communication skills
