Job Summary

Responsibilities

• Work with patients in their homes to develop and implement personalized physical therapy plans.
• Provide education and training to patients and their families on proper techniques and exercises to improve mobility and function.
• Maintain accurate records of patient progress and communicate effectively with healthcare teams.

Requirements

• A Bachelor/Associate Degree from an accredited Assistant school of Physical Therapy.
• Current licensure in state of practice.
• Minimum one year experience in a home health setting.
