What are the responsibilities and job description for the Bilingual Behavioral Health Case Manager position at ASIAN ASSOCIATION OF UTAH?
Overview: The Case Manager is responsible for assisting and being an advocate for any Client receiving services in the Community Wellness Department at Asian Association of Utah. This individual serves as a trusted source of information for the clients to gain support as they navigate life in Utah and different areas such as financial assistance, housing support, etc. Overall, the Case Manager works with high-risk clients on an individual basis to get the support they need in order to access healthcare, education, or other services. As part of a multi-disciplinary team of service providers, the Case Manager serves as an advocate for the client and assists in the client’s access to behavioral health services including psychotherapy and medication management. They work in line with AAU’s mission to improve the quality of life of refugees, immigrants, and victims of human trafficking, to reach each client’s needs and goals.
Duties and Responsibilities:
- Complete intake screening with client to identify needs and goals of each client receiving case management services.
- Accepts new referrals for case management in which diagnostic psychiatric evaluations have been completed through therapists at AAU
- Provide case management services to address SMI/SED and/or co-occurring health conditions by linking clients to resources either internally or externally through other agencies such as Department of Workforce Services
- Develop a treatment plan based on client needs.
- Enroll clients in health insurance, Medicaid, housing programs, and other financial benefits (SNAP, SSI, Cash Assistance, etc.) as eligible
- Monitor services and client well-being through routine check-ins with client
- Participate in case staffing with care providers on the team and other external care providers as needed on a regular basis to report on progress made in reaching service goals and/or client needs from other providers.
- Collaborating with other agencies, including medical providers and education services, to connect clients with local community resources
- Responding to emergency needs and/or care of patients in crisis.
- Provide outreach to clients on both telehealth and in-person platforms.
- Establishing strong relations with agencies, including local social services departments, clinics, community agencies, and schools, that serve as resources for clients
- Assisting with other duties as assigned
Qualifications:
Required
- Bachelor’s degree preferred and/or equivalent work experience in human services field
- Licensed by the Utah Division of Substance Abuse and Mental Health as Targeted Case Manager, or ability to get licensed within 6 months of hire
- 5 years of experience providing case management services to diverse populations and clients experiencing behavioral health or substance use challenges
Preferred
- Understanding of federal, state, and local social service programs and resources
- Competent with computers, Microsoft programs, and Electronic Health Record (EHR) UWITS
- Ability to speak a second language; preference for languages spoken by current refugee/special populations such as Spanish, Arabic, Farsi, Dari, Swahili, and etc.
- Competent using remote technology for client appointments
Work Environment:
- 50-60% travel anticipated

Job Type: Full-time
Pay: $19.00 - $23.00 per hour
