PRIMARY FUNCTION:
To work as a Nurse Coordinator with a team of clinicians, client services staff, interns, and leadership, to collectively deliver the highest quality of care using a client-centered, strength-based case management model, ensuring holistic care and a strong organizational culture. This role involves overseeing the outpatient withdrawal management service line, medication assisted treatment (MAT) services, and coordinated medical care across multiple facilities, with occasional travel between clinical centers, including Fort Collins and Colorado Springs, primarily in Lakewood and Thornton. This role is integral to the success of our program. You are welcoming, professional and serve as a go-to for information and assistance.

ESSENTIAL DUTIES/RESPONSIBILITIES:

Client Care:
  • Assist in coordinating patient’s medical care and access to medical resources in the community.
  • Manage and optimize the Medical Appointment Schedule and coordinate patients’ appointments with the prescribing medical provider.
  • Implement and maintain all policies, procedures, and processes that ensure the medical safety of all clients.
  • Maintain quality, confidentiality, and continuity of all required documentation and records as assigned.
  • Provide exceptional customer service and act as a liaison to all internal and external ARR customers.
Medical Management:
  • Administer injectable medications such as Vivitrol and Sublocade as needed.
  • Oversee compliance with medical policies and procedures.
  • Manage and order medical equipment and supplies.
  • Oversee medication administration procedures and supplies of controlled substances.
  • Lead and develop new programs covering withdrawal management to locations in Lakewood, Fort Collins, Colorado Springs, and Thornton.
  • Coordinate medical care between clients and prescribing providers.

SUPERVISORY RESPONSIBILITIES:
  • None

MINIMUM QUALIFICATIONS:
  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) license.
  • Active CPR/First Aid certification.
  • QMAP qualification
  • Skilled in several areas of treatment; individual, cross-cultural issues, trauma informed care, client-centered treatment, evidenced-based interventions, treatment planning, discharge planning,
  • Minimum two years of sobriety from personal substance abuse challenges (if applicable).
  • Passion for behavioral health and addiction.
  • Emotional maturity.

PREFERRED QUALIFICATIONS:
  • Experience in addiction medicine or psychiatry.
  • Leadership and managerial experience.
  • Problem-solving skills.
  • Familiarity with Federal and State of Colorado controlled substance laws.
  • Certified Addiction Nurse

CERTIFICATES, LICENSES, REGISTRATIONS:
  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) license

. WORK ENVIRONMENT & PHYSICAL DEMANDS:
  • Office and Clinical setting work environment.
  • The physical environment is typical of a standard office space. It requires the employee to work inside for the duration of work. Must be able to remain in a stationary position, sitting for more than 75% of the time. This position will require occasionally moving about inside the office to access equipment, cabinets, etc. May work around office equipment which could be noisy, such as copiers and fax machines.
