Job Description
The Scheduling Coordinator is responsible for coordinating home healthcare services to patients that include patient support, advocacy and education; to assist the patients and their families in understanding the healthcare system. The Scheduling Coordinator maintains contact with caregiver and case manager on a continual basis to ensure the client gains access for their needed physical/behavioral healthcare needs. In addition, to assisting the client in gaining access through the system and assisting with external services to provide quality patient care.
Your contribution to the team includes:
- Provide coordination of Home Healthcare services with the caregiver and client.
- Recruit caregivers and clients routinely on a continual basis.
- Actively engage in the client and caregiver to ensure the patient health plan to properly being followed
- Assist with patient outcomes by mobilizing and coordinating primary medical, specialist, behavioral health, long term care services and supports as related to the patient’s Health Action Plan goals.
- Develop and execute cross system care coordination among treating providers and authorizing entities to assist patient to access and navigate needed services and reinforce and support their health action goals.
- Connect patients to relevant community resources with the goal to increase satisfaction, patient health and well-being and reduce health care costs.
- Utilize health technology to assist in the linkage of needed services. Maintain and coordinate communication among patient/significant other, staff and physician as evidenced within medical record, client and health care team feedback to improve continuity of care.
- Assist in assuring that effective communication exists with patient/significant other as reflected in the Health Action Plan, plan of care. Work collaboratively with other members of the health care team in coordination of the patient's care.
- Assist patients, family members or other client support members with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.
- Answer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated.
- Complete Health Home contract requirements including verification of patient eligibility, scheduling and reminding patients of appointments, attending required trainings, administering and documented periodic health and behavioral screenings within deadlines.
- Meet in person and by phone with members assigned to case load at their home or location of choice when providing Health Home services.
- Participate in orientation of newly hired Care Coordinators.
Must have’s you’ll need to be successful:
- High school diploma, Associate or Medical certification in a healthcare related field
- Must have working knowledge of Home Healthcare services
- Knowledge of Word, Excel and Outlook required
- Ability to prioritize in a rapidly changing environment.
- Ability to work independently and in cooperation with others.
- Ability to work on-on-one with patients
- Must have a reliable vehicle transportation and vehicle insurance
- One (1) year experience in direct patient care or patient care support and/or one (1) year experience in care coordination, patient navigation, or community health work preferred
Job Type: Full-time
Pay: $16.00 - $18.00 per hour
Schedule:
- 8 hour shift
- Monday to Friday
