What are the responsibilities and job description for the Community Health Intake Specialist position at Assisted Home Care?
Job Summary:
We are seeking a dedicated Intake Nurse to join our home healthcare team. The Intake Nurse will be responsible for overseeing patient care, coordinating treatment plans, and ensuring compliance with medical standards.
Duties:
-This is PRN work only.
Pay is $35 per visit.
The job requires going to our client's home through out Williamsburg County, conducting an intake interview and completing a 9 page intake packet. New intakes will take close to a hour each. You will then do a 30 day follow up to see how services are going, if there are any needs that should be address and if plan of care is working out for each patient. Then you'll maintain a quarterly home visit schedule for check ups. You will also take on already established client who will only need quarterly visits. The paperwork is mostly fill in the blank. You will be given a copy of each client's face sheet for your records.
There is no hands on care required. We employee Personal care aides to see to the client's needs on a daily basis. We are a non-skilled agency so there is no BP, Glucose, temp, SPO2, wound care of any kind. You will not be doing pt care at all.
Pay: $35.00 per hour
