The DOPCS/Clinical Manager is responsible for oversight of all home health patient care services and their clinical management, and all home health personnel consistent with the mission, vision and goals of the Agency. The Director's primary functions are to plan, organize, coordinate and control clinical home health patient care services while maintaining quality. Additional responsibilities include promoting and maintaining harmonious relationships with client/families, other departments, and other community organizations.
What you will do:
- Overall responsibility for coordination of patient care services and all activities relevant to the patient care services and clinical management furnished including the development of personnel qualifications and the assignment of personnel.
- DOPCS or his/her registered nurse designee shall be available on the premises or immediately accessible by telecommunications for clinical management oversight during all operating hours when the patients are receiving services to ensure:
- Coordinated referrals and patient care.
- Appropriate patient and personnel assignments.
- Ongoing interdisciplinary assessment of the patient.
- Development, implementation, and evaluation of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s).
- Provision of services that are ordered by the physician as indicated in the plan of care.
- Provision of counseling and education to the patient, family, and caregiver(s) based on individualized needs.
- Documentation of comprehensive clinical notes in the medical record.
- Communication with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
- DOPCS shall devote sufficient number of hours to assure quality and adequacy of services provided and supervision of staff
- Management of agency personnel and contracted services. Assures that Agency personnel are performing job duties as per job descriptions and at an acceptable standard.
- Follows up with patients to assess patient satisfaction with the Agency's service.
- Directs in-services, case conferences, and staff meetings.
- Ensures that the Agency operates in compliance with legal, regulatory and accreditation standards.
- Ensures that the policies and procedures reflect quality and safe delivery of home health care services.
- Maintains and implements standards of care.
- Plans and evaluates educational needs of department personnel and promotes growth and development.
- Represents home health on various committees.
- Maintains community involvement. Maintains a working relationship with other health care agencies and groups.
- Acknowledges patient rights including privacy and confidentiality.
- Consistently displays a professional appearance and conduct in all settings. Customer services including interpersonal and interagency relationships are to be positive and professional at all times.
- Assumes responsibility and displays reliability for the completion of all tasks, duties, communications and actions.
- Supports the mission, vision and goals of the Agency.
- Participates in Agency-sponsored in-service education / training, and other professional / educational activities to maintain professional growth and development.
- Participates in the Agency’s QAPI Program for performance improvement activities.
- Abides by Agency policies and procedures.
- Accepts other duties as assigned.
- Proven experience as director of nursing or clinical manager in home health or hospice environment
- Current, active RN License in the State of California by the Board of Registered Nursing.
- Minimum of bachelor’s degree in nursing or other health-related field with three years of experience within the last five years in a hospice or home care agency, primary care clinic or health facility.
