We are looking for a hospice LVN to pick up routine visits and to pick up on-call shifts.
Routine visits- $49 per visit (including mileage)
On-call Mon through Fri- $50 per day for standby rate, plus $68 (including mileage) for each visit you take after hours
On-call weekends- $100 per day for standby rate, plus $68 (including mileage) for each visit you take after hours
(If open to cover Mission Hills as well, you can get double the standby rates each day)

As a Hospice LVN you will deliver holistic, end-of-life care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and you will effectively interact with patients, caregivers, and other health team members while maintaining standards of professional nursing practice.

- Ensure quality and safe delivery of home health nursing services.
- Must participate in on call rotation
- Communicate with RN Case Manager and physician, as appropriate, on a regular basis regarding changes in the patient's condition and/or needs.
- Assist the RN in carrying out the plan of care.
What you have accomplished:
- Graduate of accredited nursing program.
- Current, active, license as an LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.
