About the Role:
We are seeking a dedicated and compassionate Care Manager : Licensed Practical Nurse to join our team at Assisting Hands of South Jacksonville. As a Care Manager LPN, you will play a crucial role in providing high-quality care to our clients and making a positive impact in their lives. We are seeking to provide world class non-skilled care to Clay and Duval
Responsibilities:
- Assist with the development and implementation of care plans
- Provide initial and followup of ADL's and iADL's
- Collaborate with the care team to ensure the best possible outcomes for our clients
- Improve care by providing routine quality assessments of our home care population
- Assist with marketing / networking as needed
Requirements:
- Current LPN license in the state of Florida
- Previous experience in Skilled Home Care preferred
- Strong communication and interpersonal skills
- Compassionate and patient-centered approach to care
- Ability to work independently and as part of a team
