About the Role :

We are seeking a dedicated and compassionate Care Manager : Licensed Practical Nurse to join our team at Assisting Hands of South Jacksonville. As a Care Manager LPN, you will play a crucial role in providing high-quality care to our clients and making a positive impact in their lives. We are seeking to provide world class non-skilled care to Clay and Duval

Responsibilities :

Assist with the development and implementation of care plans

Provide initial and followup of ADL's and iADL's

Collaborate with the care team to ensure the best possible outcomes for our clients

Improve care by providing routine quality assessments of our home care population

Assist with marketing / networking as needed

Requirements :

Current LPN license in the state of Florida

Previous experience in Skilled Home Care preferred

Strong communication and interpersonal skills

Compassionate and patient-centered approach to care

Ability to work independently and as part of a team
