The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges, those who have suffered a trauma, and those at risk to enjoy lives of dignity and purpose.  We are looking for a Case Manager Mental Health Professional or Qualified Mental Health Professional (MHP/QMHP) in Elgin, IL who exemplifies that mission, and wants to work for an organization that makes a difference.”

Schedule: Monday-Friday 8 Hours 

Pay Rate:

Case Manager Mental Health Professional- $22.50-$25.00

Qualified Mental Health Professional- $24.75-$26.25

What will you be doing? 

• The Case Manager MHP/ QMHP is responsible for developing and implementing individual treatment plans utilizing the IM CANS to maximize the client's independent community living skills.  

• The Case Manager MHP/QMHP must complete documentation of mental health services provided in accordance with state and agency standards as applicable. 

• The Case Manager is expected to work with a team of behavioral health service providers within the agency as well as outside agencies, if applicable, to ensure positive client-centered approach.   

• This job position may have some work components that can be performed remotely. Remote work arrangements are not a right or entitlement of employment. They are discretionary and subject to demonstrated performance and operational needs. Approval may be rescinded at any time at the sole discretion of management. 

What will you bring to the table? 

• Bachelor’s degree in human services or a related field (required) MHP Rate

• Master’s degree in Human Services or related field (required for QMHP role) QMHP Rate 

• Experience working in social services (preferred) 

• A valid Illinois driver’s license and automobile insurance

• Navigation of stairs 

• Ability to lift 50 lbs. or more 

• Ability to provide services in a client’s home 

• The use of your personal vehicle and/or agency vehicles to transport clients regularly 

• Maintaining required certifications and other relevant trainings
