We are looking for a Case Manager Qualified Intellectual Disability Professional, who demonstrates this mission and wants to work for an organization that makes a difference in the community.
What will you be doing?
The QIDP Case Manager is responsible for the clinical case management of an assigned caseload. Ensures that all aspects of community living comply with all applicable Federal, State, and Accreditation standards. Responsible for developing and implementing individual programs to maximize the client’s independent community living skills and inclusion in their community. Provides documentation on assigned clients in accordance with DHS, CARF, or DPH standards, as applicable. Advocate for and maintain rights and privacy of clients that are served. Accountable for providing to both clients and staff members a positive, professional and safe environment to live, learn, and work. Responsible for a permanent caseload of up to 25, and additional cases as assigned based on the needs of the program. Supervise support staff as assigned. Provide staff training for client strategies and/or programs and assist with new hire training as assigned. Perform client case record reviews and advise QIDP on improvements as needed. Prepare a monthly accountability report as assigned. The QIDP Case Manager must be a role model for clients and staff for both permanent and temporary assignments, including providing and participating in direct service as needed. The Case Manager provides direct support as needed to meet program needs and may also work with clients who require intensive care including but not limited to toileting and bathing, special diets with personal and/or enteral feeding, and enhanced funding. This job position may have some work components that can be performed remotely. Remote work arrangements are not a right or entitlement of employment. They are discretionary and subject to demonstrated performance and operational needs. Approval may be rescinded at any time at the sole discretion of management. May work remotely for up to 20% of the work week with approval from the Clinical Manager.

Education:
-
Bachelor’s Degree in a human service field.
Experience:
-
At least one year of experience working directly with individuals with developmental disabilities.

Three to five years of experience as QIDP or QMHP preferred.

One year of supervisory experience preferred.

Climb stairs daily

No medical restrictions which would prohibit implementation of a lift/transfer or implementation of behavioral programs.

Push, pull, reach, stoop/bend over and use step stool daily

Perform daily housekeeping tasks and assist clients in independent living skills.

Physically assist clients in situations including, but not limited to: getting in and out of van, bathtub, shower, or bed regularly

Implement behavior support strategies and respond to emergency behavioral issues.

Requires significant walking and standing on a daily basis.
