Pay Range: $145 per eval, $125 per revisit

If yes, here’s how you will contribute to the team:
As the Physical Therapist (PT), you will assess, plan, and provide physical therapy services to residents in accordance with established treatment plans and goals. Your role will focus on promoting mobility, strength, and overall functional independence, enhancing the quality of life for residents, and ensuring they receive the highest level of care.
Your key responsibilities include:
- Conducting comprehensive assessments of residents’ physical abilities and limitations.
- Developing and implementing personalized physical therapy treatment plans in collaboration with the interdisciplinary care team.
- Providing direct therapy interventions, including exercises and manual therapy techniques, to improve physical function, strength, and mobility.
- Educating residents and their families about the therapy process, including exercises, mobility techniques, and self-care strategies.
- Monitoring and documenting patient progress, adjusting treatment plans as necessary.
- Ensuring compliance with all state and federal regulations governing physical therapy services.
- Collaborating with other healthcare professionals to integrate physical therapy into the overall care plan.
- Providing training and guidance to physical therapist assistants (PTAs) and other staff as necessary.
- Advocating for the needs and well-being of residents, promoting a person-centered approach to care.
Must Have:
- 1-3 years of experience as a Physical Therapist.
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- Current and unencumbered Physical Therapist license in the state.
- Knowledge of physical therapy principles, practices, and techniques.
- Experience with long-term care environments and familiarity with relevant regulations.
- Strong interpersonal and communication skills, with the ability to work effectively with residents, families, and staff.
Nice to Have:
- Experience in a Skilled Nursing Facility.
- Experience working with elderly or disabled patients.
- Familiarity with Medicare and Medicaid therapy regulations.
- Advanced certifications in specialty areas (e.g., geriatrics, neurology).
