Job Summary
We're seeking a highly skilled Registered Nurse to join our team as a Home Health Nurse. In this role, you'll work closely with patients and families to deliver comprehensive care plans, ensuring they receive the support and resources needed to achieve optimal health outcomes.
Responsibilities:
- Conduct initial and ongoing assessments of patient needs, developing personalized care plans that address medical, emotional, and social requirements.
- Collaborate with physicians, therapists, and other healthcare professionals to ensure seamless coordination of care.
- Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
- Document patient progress, identifying areas for improvement and implementing changes as needed.
Requirements:
- Curent RN licensure in state of practice
- Curent CPR certification
- Curent Drivers License, vehicle insurance, and access to a dependable vehicle or public transportation
