Office Hours: 9:00am -5:00pm (Monday-Friday) with an hour lunch break

QUALIFICATIONS:
Bachelor’s degree with major specialization in social, behavioral, or human services or
related field; high school diploma with a minimum of two years full-time work
experience in social, behavioral, or human services or related work; or license by the
State of Texas as an LVN or RN with one-year experience in human services. Must
be proficient in the use of Microsoft Word and Excel.
PURPOSE:
This position coordinates services for individuals enrolled in the Home and
Community Based Services (HCS), Texas Home Living programs (TxHmL), and
Department of Family and Protective Services (DFPS) operated by Assured Quality
Care Services, LLC. Program Coordination is available as determined by individual(s)
need. As a Program Coordinator, the position can serve an unlimited number of
individuals.
ESSENTIAL FUNCTIONS:
1. Coordinating the development and implementation of each individual’s IP’s/
Support Mythologies:
2. Coordinating the delivery of each individual’s IPC;

3. Coordinating and monitoring the delivery of HCS and TxHmL Program
services and services from other sources;
4. Monitoring/Recording each individual’s progress or lack of progress.
5. Record keeping, to include maintaining in-service records and the
Individual’s primary record.
6. Develops, implements, reviews, and updates all individuals’ training data sheets,
monitoring sheets, and recreation sheets based on the individuals Person
Directed Plan (PDP)/IP’s/ and Support Mythologies.
7. Completes/Monitor’s reviews of all HCS/TxHmL participants’ PDP as
determined by the Program Director.
8. Completes IP’s/Support Mythologies for all PDPs and Ensures each
individual’s PDP is a current and is an actual reflection of their needs, goals,
assessments, and desires.
9. Works with financial manager to ensure individuals’ funds remain below the
designated limit and completes financial reviews as requested.
10. Ensures documentation including but not limited to service delivery
logs/progress notes, training and behavior data sheets, monitoring sheets,
recreation sheets are provided to direct care staff, respite staff, community
support providers, foster care providers, and other HCS/TxHmL service
providers as necessary on a monthly basis.
11. Coordinates and/or Completes individuals’ appointments with the Social
Security Administration and Department of Human Services as necessary
and/or provides documentation as requested.
12. Arranging and/or providing transportation.
13. Assists individuals with opening bank accounts as necessary.
14. Coordinates the purchase of adaptive aids and durable medical equipment
through Medicaid/Medicare and the HCS/TxHmL waivers including the prior
approval process as necessary.
15. Coordinates obtaining minor home modifications including the bid process and
prior approval process through the HCS/TxHmL waivers.
16. Schedules and facilitates provider service planning meetings as necessary.
17. Participate in on call/AOD rotation.
18. Responds to the emergency needs of the agency.
19. Attends/Participates in annual individual planning meeting. Also attends and
participates in interim staffing when necessary.
Position Requisition
20. Communicates changes in the individual’s needs to the assigned service
coordinator.
21. Completing/Submitting MR/RC and IPC to the Program Director by the
deadline date determined by the Program Director.
22. Complete monthly or quarterly face-to-face visits with all individuals served in
the HCS/TxHmL waiver programs or as deemed necessary by the Program
Director.
23. Coordinating/Providing in-service training to direct care professional staff and
foster care staff on the individual’s service/training/special needs. And
maintain record of said in-services.
24. Completing ICAP’s every three years or as needed on the individuals served in
the HCS waiver program. To include submitting the required documentation to
HHSC Utilization Review Department in the required time frames for
individuals requiring and LON increase/ behavior bump-up.
25. Files PDPs, IPs, Support Mythologies, IPCs, ICAPs, MR/RCs, and progress
notes in the primary charts.
26. Completes Emergency Plans.
27. Coordinate/Implement DFPS services according to state guidelines.
28. Completes all other duties as assigned.
TIMELINE REQUIREMENTS:
1. Completes IP’s (Annual and Interim) are due 14 th days from date received from
the MRA/Service Coordinator or PDP mgt.
2. Bi-Annual and Quartiles review reports are due on the 15 th day of the following
month.
3. IPC due 30 days prior to the IPC end date.
4. IDRC due 45 prior to the IDRC expiration Date.
5. ICAPs due every 3 years by the individuals anniversary dates or when there is a
change in the individual’s level of functioning.
6. Progress notes both face to face and collateral contacts are due on the 10 th day
of the following month.
7. Reviews training data, monitoring sheets, behavior data, and
recreation/community integration sheets monthly and updates all necessary
items by the 30th of each month.

Pay: $14.42 - $24.04 per hour
